We recognize that transitions can be difficult for a child with SEND and take steps to ensure that any transition is as smooth as possible.

How are children prepared for moving on?
Nursery/Pre-School to Reception
Transition for our Pre-School at Tanfield is seamless as the children are already within the Reception setting. Those children who attend other providers will have visits from our staff during the summer term to enable our staff to meet your child and discuss their needs. If any additional provision needs to be made we would contact you.
Transition between year groups within school – prior to the new academic year (September) children will have the opportunity to meet their teacher and have time in the new learning environment. For children with SEND additional visits can be arranged and where necessary further work will be done with the child to aid this move.
Transition from Primary to Secondary
The majority of children move to Tanfield School. From February half term a transition worker (Mrs Rockett) is timetabled to support in Y6 on a weekly basis to enable them to get to know the children, be aware of any barriers there may be to a smooth transition, and put plans in place to aid this. Transition visits take place at the beginning of July. Children who are more vulnerable are identified and supported during this time. If additional visits are required, these will be arranged. The SEND at both schools will meet with the parents to discuss individual cases.
